Once completed, the form can be submitted via email, fax, mail, or dropped off in person. Bethel Park charges $.25 cents per copied page for all standard paper copies; additional fees may apply for non-standard items. Emailed/scanned copies are provided at no charge. Exact postage is also charged if the records are sent via U.S. Mail.
